1969 Capitol Record Club edition with colorband labels.
The cover has three black bars on the top corners and has the number "16" on the back.

The print at the rim of the label states that the records were "Manufactured under license from Capitol..." rather than "Mfd. by Capitol." 

This record club edition features true stereo versions of 'Dr. Robert', 'I'm Only Sleeping' and 'And Your Bird Can Sing'.  These songs were reprocessed stereo on the original LP and some reissues.
